Open Records Request Form

If you have a request for a STUDENT RECORD, please email Kaila Steele for assistance at kaila.steele@mymec.org.  This Open Records Request Form is not for student records.  

For your convenience, to submit an open records request, you may do any of the following:

1. Download the Open Records Request form and submit it via U.S. mail to Mountain Education, ATTN: Elisabeth Shaw, 1963 Tom Bell Rd. Cleveland, GA 30528
2. Download the Open Records Request form and deliver in person to Mountain Education Central Office, 1963 Tom Bell Rd. Cleveland, GA 30528
3. Download the Open Records Request form and email the completed form to Elisabeth Shaw at elisabeth.shaw@mymec.org 
4. Open the online Open Records Request Form, fill out and submit online.   MECHS will be notified of your request.  
5. Submit your open records request by phone by calling Elisabeth Shaw or Kristin Jarrard at (706) 219-4664
